self-discipline at its core is nothing more than a promise you make to yourself. It's you vs. you and it starts with a commitment. Do YOU want it (whatever it is) bad enough that you are willing to commit to it?

One small step
Few things in life happen because you're just so damn talented at it. Just like anything else, this skill is like a muscle. Weak at first, but start with just one pushup, one step, one word, one stroke, but just start.
Whatever it takes
Use little reminders and any tools available at your disposal to stick to your action plan. Sticky notes, apps, calendars, motivational posters, funny mugs, whatever it takes to stay the course.
